Filing artifacts by state
A birth worksheet is a pre-filled state filing artifact generated from a birth record — a draft of the paperwork your state’s vital-records office expects, populated with the detail you already captured. An attendant reviews it and signs it, and we never file on your behalf: nothing leaves the registry until a person has checked the packet and attested to it.
Where filing lives
Section titled “Where filing lives”Worksheets hang off the record, not the Births log. Open a record from Records to reach its detail drawer; two actions there drive filing:
- Download forms — the pre-filled, DRAFT-marked worksheet PDFs for the record.
- Review & sign — the human gate, where you preview the packet and sign it.
A record has to be Locked or Amended before it can be signed — the states whose attested content can no longer drift out from under the signature. A draft record opens the sign page to a “not ready to sign” notice instead. See Review & sign for that flow.
Supported states
Section titled “Supported states”The registry can currently produce filing artifacts for three states. More are added over time.

The Washington parentage affidavits are only offered when the birth’s parentage determination calls for one; a married or unrecorded marital status leaves them out of the menu. A Vermont filing for an unmarried parent also completes a Voluntary Acknowledgment of Parentage (VAP) for the town clerk / Vermont Vital Records; a married parent relies on the marital presumption and files no VAP.
Downloading the worksheets
Section titled “Downloading the worksheets”From an open record, choose Download forms. The menu lists:
- WA worksheet (draft PDF)
- OR worksheet (draft PDF)
- VT worksheet (draft PDF)
When the parentage determination requires an affidavit, the two Washington artifacts join the menu below the worksheets:
- WA Acknowledgment of Parentage (422-159)
- WA Denial of Parentage (422-158)
Each item is a pre-filled draft PDF, watermarked so it can’t be mistaken for a filed form. Review it, then sign on the record’s Review & sign page.
Reviewing and signing
Section titled “Reviewing and signing”Signing is a write action, so it’s available to Provider and Admin roles, not the read-only Viewer. Because a signature binds to the record’s current version, correcting a signed record cuts an amendment that must be re-signed. The Review & sign page walks through the artifact picker, the packet preview, and the signature history.
