My birth history

My birth history is your own career log of births — every birth you are the attributed attending provider of, across every practice you have ever worked in, newest first. Open it from My birth history in the sidebar.

It exists because a provider changes practices over a career while BirthTracks stays the constant: your attendance history has to survive a practice membership ending. A birth you attended at a former practice stays in your log, marked Former next to that practice’s name.

What a row shows

Each entry is one attended birth:

• Date — the date of the birth (Undated when no date is on file).
• Practice — the practice the birth was recorded in, with a Former note when you no longer belong to it.
• Role — your role on the birth (Attendant when none is recorded).
• Site, Delivery, Outcome — the birth’s place, delivery mode and outcome.

Where the birth is in a practice you can still access, the row offers Open chart, which hands off to the ordinary chart — with its own access check and audit trail. For a birth at a former practice there is no chart to open, so the row reads Log line only. When you have no attributed births yet, the page reads No attributed births yet, with the note that births recorded with you as the attending provider appear here — and stay here if you move practices.

What it is not

My birth history is a log of your attendance, not a window into patient records. By design it carries no patient identifiers — the privacy boundary is drawn at the log itself, so the page could not show a patient name even if it tried. To see the full record of a birth, use Open chart on a birth in a practice you currently belong to.

Available without a subscription

My birth history is the one clinical surface that sits outside the subscription paywall. It is not tied to any one practice, so there is no practice subscription to check — only that you are signed in. Your career log stays reachable even when a practice’s subscription has lapsed. (As a read surface, it is off-limits to a write-only Scribe seat.)
